The family is your employer. One of the hurdles they have to get over in obtaining your work permit is the provision of medical insurance. They should know that.
Other au pairs may correct me, but I think it is usual for the employer to pay the medical insurance and then deduct a % from the pay check.
I do not think that travel insurance will be enough.
