You should not give them any excuse to ignore your cancellation. Don't go into the reasons, simply say you are giving the required notice to cancel by such and such a date (and name that date). Don't forget to send it registered, and keep the receipt in a safe place.

Here's some advice I once came across about cancelling broadband services in the UK:
  • Don't use obscenities, as that will allow them to file your letter in the bin.
  • Don't include any moans or accusations as that may give them a means of saying that they have "improved things so have cancelled your request to terminate the service".
