They needed:
1) a copy of my insurance card
2) A letter from my Boss saying I was not an employee (or something like that). Acknowledgement from the training center

3)
Originalformular B (Foreign insurer's confirmation for the purpose of exemption from compulsory healthcare insurance in Switzerland)
4) The original bill showing the dates covered.
I think it also might have helped that my boss's secretary handled the calls.
