No you don't. You will fill in an annual tax declaration.
You enter as your main income what you have earned with your employment contract (Haupterwerb) and you also enter what you earned being self-employed (Nebenerwerb). Whereas your main income (Haupterwerb) is where you earn most and where your working hours are longest. E.g. you are amployed 60% then your Haupterwerb is your employment. If your employment contract is 40% then your Haupterwerb is your income from sel-employment.
Your employer normally pays taxes, pension fund contributions and deductions for you. From your self-employment part you must pay contributions to the AHV(AVS) yourself. You must notify them that you are intending to work free lance and they will send you a bill; which they normally do once in a quarter.
There is no mandatory insurance ot deductions from your income as self-employed apart from taxes (based on your declaration and AVS (AHV).
Actually, if your employment contract is less than 100% your employer is not allowed to forbid you to work else where, unless you are a direct competitor to the company that employs you. However, if this is not part of the contract then you are free to act as you wish.
