Getting a Einzahlungsschein

So I need to get some money back from my health insurance and they want my bank info and an Einzahlungsschein enclosed (with some other details as well).

Can I just go to the bank (UBS) as ask for one? Or do I need to order these?

any quick answers~!?

Usually there is a form to request for the EZ from your Bank - tick a box for 10, 20 or more pieces. I do it online.

So I have to go in to do this? or can I order them from online banking (quick look around the website hasn't shown anything promising)

I don't use UBS, but I guess you can order it online ( NetBanking ).

You'd need to order the one without a reference number ( that is, a general purpose deposit slip ).

I got some recently from the Post for this purpose. They're completely blank and you can fill in your own details.
