Cancelling a UBS Maestro card

Anyone have a link to a form for cancelling a UBS Maestro card? I checked out the UBS website, but of course there is no obvious option on how to cancel any of their services. It's my wife's card for my UBS account that I only rarely use as an additional account.

Cut the card n half, send it to the bank with a letter cancelling the card.

I suppose I could do that. I was trying to be lazy and not have to draft a letter myself.
