Sub-Renting my Apartment

Hi,
I'm leaving Switzerland for a semester and don't want to give up my apartment (yup, finding one in Zurich is next to impossible... let alone at a good price and a nice location), so I've decided to rent it out. I have the yes from my landlord and found a nice person who is interested.

It is all down to the contract now, and I really don't know how to handle something like this. Should I simply copy my own contract and put an extra paragraph underneath that limits the time, or is there something else I need to consider? Maybe there is a "template" that someone has?
