2012 Sharing of medical data with insurance company

I see today on Google.ch a number of articles about change in data sharing between doctors/hospitals and the insurance companies. What additional information will be shared? Will all of the doctors notes and conversations be shared? Will I be able to opt out of this? I did a search on the forum and did not see this discussed before but sorry if I missed it.
