Pregnancy and PhD student health insurance

I am a foreign PhD student in canton Vaud married with a non-Swiss citizen. I am looking for a health insurance which would cover possible future pregnancy.
1. Does anyone have experience with Swisscare in this regard?
2. Could you recommend it or other good insurance companies for PhD students which cover all pregnancy costs?
3. I read that Swisscare only accepts PhD students earning less than 42k in canton Vaud. Does this apply only to my PhD salary or to my family income (together with my husband's salary)?
4. Do they cover the pregnancy costs immediately if the pregnancy starts after signing the insurance contract? Do I have to have been their client for a certain time before being able to claim pregnancy coverage? Are there any other caveats?
5. Is Swisscare accepted in hospitals of canton St. Gallen?

I can answer only q3. Yes, your PhD salary needs to be below a certain amount (determined by the canton) to permit registering with Swisscare. Your joint income doesn't apply.

Swiss health insurance will cover you immediately, in fact it's usually backdated to the date of your arrival in Switzerland so any medical bills you incurred before you got the policy should be able to be claimed back.

Why enquire about cover in St. Gallen when you're based in Vaud? Swiss health insurance cover is recognised by all hospitals, but depending on the type of policy you take out you may be restricted to certain doctors/hospitals.

Note that, from what I can see on Swisscare's website, you have to pay up first yourself and then claim the costs back from them. This may not be ideal for you.

You have to have the same cover as the basic LaMal Swiss health insurance so whichever insurer you choose will cover you for pregnancy.
Swisscare is accepted in some cantons and under certain conditions and will provide the same level of cover as Swiss basic insurance if you are eligible to use them so pregnancy will be covered although I'm not sure that they will cover all the costs if you decide to give birth outside of your canton if residence.

Any health insurance cover has to be approved by the canton of residence and there are not many non Swiss insurers who are deemed acceptable.
