The proper operation of these kinds of things requires that the bungee usage be logged and that the cords can be retired after a certain predefined usage. The reason for this is so that these types of incidents don't happen.
I would say that the claimed 1:500000 failure rate is a bit optimistic if the company has had around 50000 customers not all of whom could have possibly used the defective piece of equipment.
Point is that the bungee cord didn't "just fail," unless it came from the factory that way. When things like this happen it is at least three things that have "gone wrong."
Statistics don't protect us if we don't exercise due diligence.
Happy to see that she seems to none the worse for wear. That will be a story for the ages.
