My partner and I did this in Zürich and the form was completely different. I'm kind of taken aback - although I can understand why - about the part asking how you met - kept in touch - etc.
For me - a US citizen - once we completed the form I had to have an original birth certificate issued within 6 months - a copy of my passport - a notarized residency statement (showing the my current official residence) was outside of Switzerland at the time - and of course the 300CHF or so in fees. However - we made sure we had all of this information prior to going to the canton office and so when we completed the application they reviewed our documents - made copies where necessary and approved the application on the spot. We actually then chose the date for our civil ceremony that day - and were done.
All in all it was a pretty easy process -- I know -- a shock, right
Now we are in Schaffhausen - I applied for my B permit and it was issued 7 days later -- of course it took another 10 days for them to notifiy me it was ready - but overall pretty smooth sailing.
