Leaving Switzerland, form?
I have a B permit and leaving Switzerland for good.
Is there a form I need to leave so I can show my insurance I left the country for good?

Your commune can perform the Abmeldung ( deregistration ) and issue you a confirmation which can be shown to eg insurance, telco and other parties.
