It's not available online - you have to apply at your Kantonal passport office.

Or through the Swiss Embassy (I noticed you are in Canada). Contact your nearest Embassy or Consulate and they will provide the forms for you. That's what we did when we were living in London.

I received via email today from the consulate. I wonder why they don't make this available online? A bit odd nowadays.
