Seeking information regarding picking up entry visa from Washington, DC Embassy

Hello. Does anyone have any recent information regarding the necessary process and documents required to obtain an entry visa from the Swiss Embassy in Washington, DC?

I have already received a notice from my future employer in Zurich that my Ermachtigung Aur Visumerteilung (Entry visa D, I think?) is currently at the Swiss Embassy in Washington, DC. And, from the information I found on the embassy's website, it appears that I can send a self-addressed, stamped envelope containing:

(1) Original and copy of my passport
(2) 1 recent passport photo
(3) Authorization letter from Switzerland
(4) Visa Fee ($60 USD)

Can anyone confirm that this information is correct? And, if so, approximately how long will it take to receive my passport back with the entry visa?

Any information from someone who has recently been through this process would be greatly appreciated. Thank you.

That looks correct, but there should also be the visa application form - right? I think ours took around 3 weeks, but that was longer because we had included the wrong fee and had to send in a second money order (so don't worry about problems: they'll call you if there is something missing and it can be quickly fixed). If you're concerned about the passports getting lost or the turnaround time in general: pay for a couple of flat-rate prepaid priority mail envelopes and take note of the tracking numbers on both so you can watch for when it arrives and when it is returning. There are 2 sizes of the flat-rate envelopes that one will fit inside of the other (a document size and smaller letter envelope size perfect for returning a passport).

Thank you very much for your reply. Yes, I forgot to include the visa application form. And, yes, I will use priority mail envelopes with tracking numbers.
