I don't mean to be unkind, but I think a bit more careful thought about what precisely you want this survey to measure would make a big difference. For example, question 15 strikes me as a little absurd:

I mean, the answers to nearly all of these would pretty much depend on
what the new product is, don't you think? I can't see how anybody can give a meaningful answer to the question as you've phrased it here.
