Regardless of what with hindsight and more digging true amount is now, the message was that they want to use the money for struggling areas of the UK services. They also did not say it 'all' would go to the NHS.
Seems clear here that they did say "all to the NHS".

Maybe cynical people like you and me did not believe but many voters would have believed it.

"It was hastily taken and fact-checked." Cameron announced the referendum date on Feb 18th so they had plenty of time to prepare an honest campaign.
