I recall a marketing psychographic from some seminar:
Those who are at the top are motivated by the desire to differentiate themselves from the pack. Those below the top are motivated by the desire to emulate those at the top.
Trump never really needed to strive for social acceptance in NYC. He can say all kind of dumb things, and people still sought him out. So he never really needed to rely on charm and appeal, and has not refined them. Contrast with Obama who needed charm and appeal to advance.
