Yes, I've had that exact problem over the last few weeks (and reverted to IBAN) but it seems to work now (the last 3 top-ups worked as intended).
It does ask for the CVV number each time though, which is new for me.

There was a very publicised incident of someone's credit card used fraudulently beyond its monthly limit, and I speculate that Revolut and/or UBS tuned their systems to prevent that, therefore the bug and the CVV prompt.
