When you file taxes do you get such a form anyway?

Who would I ask for this form? The Bern canton tax authority, the Bern city tax authority or the government tax office?
You can find this info on your monthly paycheck or the annual income statement you get from your company. States how much the employer on your behalf paid for tax and social security.
