Unfortunately this is just a trailer... sorry, I couldn't find the full documentary. I hope someone else will link to it; it's impressive.

EDIT: This was in Switzerland, through Dignitas. Dignitas helps people from abroad, as well as those who live here.
For those who live in Switzerland, though, and for Swiss citizens who live abroad, assisted suicide is also available (after the thorough ethical checks) from EXIT, for considerably less money.
