If you live in the City of Zurich get in contact with the Städtische Gesundheitsdienst and get a letter where it states that your application has been received and is being processed. (You actually should have got such a letter automatically). Forward a copy of this letter to SWICA. Then you fall under "oder ein Gesuch um Befreiung einreicht." category.

If you live outside contact the Kantonale Gesundheitsdirektion to get the same. Procced as above.
