Translation needed for VAT form

Hi all,
I am registered here for VAT and have made 2 vat returns. But, I didn't fill in the VAT form
The relevant office has been in touch to ask me to complete and return it. I have calculated the VAT correctly. However, I am struggling with filling in the VAT form. I need to translate it into English. However, the thought of a few hours on google translate fills me with dread.
Any EF member out there done this already?
Hoping that someone out there has an English version they could send me. I would be most grateful!

Perhaps you need a Swiss accountant? A retired one could be found on rentarentner.ch

Thanks Fatmanfilms. Yes indeed I do, but dont' have one at the moment.
However, that site is a super idea and I will look it up.
