RAV expiry - BVG Risk insurance health questions

My Rahmenfrist will soon end and I was advised by my RAV lady to extend the BVG Risikoprämie within 30 days after the end of this period.

I found the website -http://www.chaeis.net/alv-arbeitslosenversicherung/freiwillig-versicherte/anmeldung.html

which eventually leads to a registration section which includes a health questionnaire form - Gesundheitsfragen VP.

This basically asks about any problems in the last 5 years - all hospital stays, diabetes, heart issues, epilepsy, what tablets, etc. , which in my case does not make good reading.

Currently, the Arbeitslosenkasse pay 39 CHF per month for this disability and death insurance.

My questions are :-
1) if I send all my medical details to these guys within the 30 day timescale, would the premium still be the same or would it increase based on my health issues.

2) Also once I have provided these details, would this affect me in the future if I worked for another employer i.e. would they insist on extra payments from a new employer instead of the default, because they now have my details?

I don't want to shoot myself in the foot, but if I do not provide all details, then I would not be covered if I need to claim the insurance - which could be a distinct possibility.

I have asked a few friends who have their own small businesses, but they are not too sure.

Accident insurance for up to a further 6 months is available from SUVA for 25 CHF a month. You need to choose how many months you want it for as you can't extend & pay for it in 1 lump. The form is available from the RAV, there is no health questionnaire to fill in.

Accident insurance can be added to your existing health insurance for less, however your salary is not insured, with SUVA salary is insured.

I already have the accident insurance organised with the Krankenkasse and am in on-going process of the IV application.

What I do not understand is what I would lose if I do not continue the BVG insurance - I think that it is the difference between Invalidity and disability, but what is not clear is how much this would mean financially at the end of the day and would an on-going BVG contribution be worthwhile?

Also, I still worry about getting a black mark if I send my medical records to the Stiftung Auffangeimrichtung BVG

As you probably know, there are two parts to BVG: savings for old-age pension and insurance against death and invalidity. This is the way the BVG insurance part worked at a former employer:

1) Life insurance up to the employee's insured salary.
2) Invalidity/ disability insurance, after two years, up to 80% of the insured salary.
- For the first two years, a separate short-term disability insurance policy covered up to 80% of the insured salary (this policy was under the KVG law, i.e., not BVG).

As I recall, the BVG insurances and KVG short-disability insurance at this company did not require a physical examination and no or possibly limited questions about a person's health.

There was also a management pension/ insurance scheme for salary amounts that exceeded the insured salary under the above BVG plan. Entry into this policy, which had a small number of members, required a physical examination and a health questionnaire.

That is the way it worked at one company.
