We just gone through the same process and all that was required was:-

1. Signed form
2. Two copies of UK passport compliant pictures, but one with counter signature at the back. I would highly recommend everyone to spend time and the extra cost to get this done professionally because you'll have to look at that picture for the next 5 years.
3. Original copy of birth certificate with at least English translation
4. Original or certified full copy of the birth certificate of my wife as she was born in UK and I was born in some remote village in a third world country. I didn't originally send this but they insisted for this document because they wanted to know my wife's parents details. I still don't understand how this would help my child's citizenship?

Once all the info is sent, it normally takes 2 to 3 weeks for everything to come back to Switzerland.

Thanks Meister - it's that birth certificate in English that's the sticker. We will order one of the international ones.

We received our documents (birth certificates of Mother and baby) about 10 days after we sent them. They were sent by DHL. We received the passport a few days later (again sent by DHL). I have been pleasantly surprised by the efficiency of the process (normally these things require a dose of beta blockers to steady my pounding heart)
