From my personal experience, I can recommend you http://www.iroc.ch/
I had the surgery done by Dr. Theo Seiler about 3 years ago. No complains so far..

I paid CHF 8'400. this is not counting the check-ups afterwards. the insurance paid nothing, but i was able to get a small tax deduction because of this (adding other medical bills from the same year to this and deducting them all off my income).
i know that it is a lot cheaper abroad. i just thought i'd rather take no chances and have a really good local doctor do it.

I would go to London, it's far cheaper even with travel.
I paid £1450 (per eye) for top of the range treatment.
Take a look at http://www.accuvision.co.uk/
Their surgeon has a private practice in Harley Street, & has been a consultant at Moorefileds & writes many papers, he is about as good as you can get.
They do not try to sell anything, you go to see them for about an hour, they give you an envelope with the information & costs. They don't call up, it's 100% up to you to make the decision to go ahead.
Edit,
I only wanted 1 eye done, the Swiss Dr insisted on doing both, as the other eye has perfectly good vision well above driving license standard, accuvision were quite happy to leave it alone. They said that there was medically no reason to touch it, the potential benefit to me would be close to zero.

I had mine done in Dublin, paid EUR 550 per eye three years ago. Thoroughly happy with the results.
The only catch is that they do want you back for several post-op checkups (mine were 1 day, 1 week, 1 month, 3 and 6 months) so it needs to be a city you'd regularly visit anyway, else the travel costs start to stack up.
Still probably cheaper than having it done in Switzerland though!

They only check the vision & see if your eye is too dry. You can get this done locally if you want it's only a 5 minute check up.

I worked it in around Christmas vacation to the US - made an overnight stop in Dublin each way, that was the 1 week and 1 month checks sorted. Went back at 3 months (I was traveling to Dublin frequently anyway at the time) and phoned in the 6-month checkup... literally "Sorry, it doesn't look like I will be in Dublin this summer. How important is this last checkup and can I get it done locally where I am?"
"How's your vision? are your eyes dry at all? Sure you're grand then. Go and see a local doctor if anything changes."

I had my eyes done about 2 months ago (both eyes were about -2.5) - best decision I made.
Did a lot of research in near by countries since I did not want to drop 8k CHF, until I found a place I was comfortable with. Ended up going to Koln, Germany, good clinic, professional, and quick.
Feel free to PM me if you have any specific questions.
Cheers,
