Re the metal nose piece - you can use the closure from something like a bread bag. I've stitched a piece of bias binding to the inside of any cloth masks we had that had no metal in them, leaving one end open in case the metal needs to be replaced. I opened a the seam of a fabric mask I'd bought with a nose piece included, it was exactly the same as they use to close bread with so I save them up
I also bought us some very funky disposables from momask.ch

I have no doubt that the number of respiratory problems will increase rapidly over the next few months DUE TO the wearing of masks.( and not due to covid)

I have no doubt that fewer people are likely to get colds and flu and the resulting respiratory complications whilst we are all wearing masks.

Anecdotally, I can tell you that as an asthmatic I breathe better without a mask than with. But I wear it anyway and try to get regular fresh air when possible. When I flew to the States, I had to switch to a paper mask because the 3-layer cloth made it too difficult to breathe in the lower oxygen environment. YMMV

So a completely evidence or fact-free opinion.

Would be actually fairly easy to test historically. Surgeons have always worn masks for procedures and for longer portions of the day than you or I would have to even now - I'm not aware of them having above average respiratory problems.

Same gives for dentists.

It would be fully unrealistic to think that the wearing of masks has no negative health affects
