Covid-19 Vaccination at Baar Centre

Hi all,

last night I received a text with my first and second vaccination appointments ... hurrah! It's at Impfzentrum Zug in Baar.

Does anyone know what I need to take along; I assume insurance card (SWICA) and residency permit (B).

Has anyone had their jab there...what was the process and will my very bad German be enough?

If you have the yellow, international vaccination certificate (or an other one) take that along.

I have no medical documents or "yellow card"... but I called up and was sent this info for Baar.

Please note each centre has their own way of working/rules:

You will be met at the entrance control point of the vaccination centre. You must present your personal identity card and your appointment confirmation. You can then enter the vaccination centre and go via reception to the check-in, where you present your health insurance card.
Any other administrative details will be clarified at that time. You will then be asked to go to one of the vaccination cubicles where you will hand in your written Declaration of Consent (see 7. Declaration of Consent) as is usual for vaccinations. If you have not done so already, you will have an opportunity to complete the corresponding form in the vaccination centre.
You will now receive your Covid-19 vaccination. You cannot choose between the approved vaccines.
Following the vaccination, you will go to the waiting room where you remain for at least 15 minutes under medical supervision. After this time, you may leave the vaccination centre. You should expect the vaccination appointment to take about 40 minutes.

Now this is interesting. Where can one see that "declaration of consent" as it is not usual for vaccinations! Here (found UK and Australian forms).

The text is pretty clear, OP. Just follow that advice.

FWIW in SG all the details including consent were taken care of in the online registration. At the centre you simply needed to show your QR code for scanning. But every canton is different.

Hi Wheatism,

super easy, super quick, super friendly. AND, they will all speak English with you if you want and have the forms in English too if you ask. Insurance card... "permit" was not needed. Do take the vaccination card if you have, but not mandatory. they will give you a print out certificate that you had the first shot.

The declaration form is available to download and fill in in advance here:

Apparently they also have them "on site" if not filled in in advance

I got this info from the Zug Corona-19 Vaccine website:


• Health insurance card
• Personal identity card
• Appointment confirmation
• Medical confirmation form (if required)
• Vaccination card (if entry is desired)

Does anyone know where to get a paper yellow vaccination card? I know it's an official WHO document and reading threads I see lots of people have them... but I don't!

Amazon seems to sell a few different ones for about €6-€25 but I'm not sure if they are "real"...

As I read that the electronic certificates are "delayed" due to security issues (via The Local) I guess a paper one will do as a backup to the receipt given...

You can get one at the pharmacy, they charge around 1.50chf for them.
