Questions about tax declaration: stocks and cryptos

I'm doing my Swiss Tax Declaration using the Private Tax application and have a few questions:

1. Do I need to declare stocks / cryptos sold before the year end?
2. My broker has a Stock Yield Enhancement Program where they paid me interest on my stocks lent to their clients for short-selling purposes. How do I declare this income?
3. Are purchase dates required to be input for all stocks / cryptos?
4. Where do I declare the free cryptos received (e.g. KICK, NeoGas)?
5. What happens if I make a mistake in terms of putting income (e.g. from 2. and 4.) in a wrong place on the tax form? Full taxable income would be declared in any case.

Opinions:

1. Yes
2. Consider classifying on WV as: Vergütungs-/ Ausgleichszins
3. Yes, but it might also be possible to attach a Depotkonto listing to save work
4. It would seem that the "free crypto" could be dividends with a value and should be classified as such. However, if they are similar to a stock dividends, where more stock is simply issued on the same capital, then possibly under WV Krypto. More information would be needed and additional research should then be done.
5. A difference between category 2 and 4 income on STE is that 2 self-employed income is subject to AHV while 4 income from securities is not.

Thanks Mullhollander, I'd like to follow up on a few things:

2. After I select Vergütungs-/ Ausgleichszins on WV the field for Ertrag mit Verrechnungsteuer is grayed out. Are you sure that I do not need to pay tax on this income? Here is more information on the Stock Yield Enhancement Program:

This is definitely not self-employed income. All I had to do was allowing them to borrow my stocks in the account settings. There was no other action from my side. They pay interest monthly.

4. NeoGas is indeed similar to stock dividends. Should I declare its value as Ertrag mit Verrechnungssteuer?

On the other hand, KICK was granted to me for free without myself knowing about it. Here is the story:

What would be the best way to declare it?

6. Private Tax application is showing exclamation mark next to the DA-1 entries. I have declared some US stocks but they have not paid any dividends last year. Can I just ignore it?
