I wouldn't say it was optional, more that they don't cover it for everyone.

I know people on unemployment benefits who don't get it paid for.

For the benefit of those who don't speak German, please can you explain what RAV means?

The RAV is the local unemployment office.
It's not paid by the RAV, it's deducted from the payout so paid for by the unemployed person.
