The health insurance companies don't give a discount. But the municipality (Gemeinde) or the canton can subsidise the premium. They do so by paying a small slice of the premium on your behalf, directly to your medical insurance company, and then you have to pay the rest. There are different levels of subsidy, depending how low (on their table) your income is. Like so many things, this can be different from canton to canton.
The vocabulary in German is "Prämienverbilligung" (premium subsidy).

If you can't find the name of the office, then you could ask your medical insurance to tell you (according to your address) which government department is responsible.
