Are your medical documents sent to your health insurance company?
 
I visited a specialist for a consultation and they mailed me a very detailed summary of the visit; it was three pages long and included a full written explanation of what happened during the visit. Will the clinic I visited send the document to my health insurance company as well? I've never been sent such a detailed summary and to be honest I don't know *how* my health insurance is billed (that is, are they also sent personal documents of my visit, such as the one I received, etc.).

No. Unless there is a dispute of cover.

You get an itemised bill that is codified by activity that justifies the billing. That's what the insurance company gets by default.

No. That is your part. The "Rückforderungsbeleg" is what you send in - if the clinic didn't send the bill directly. If they did, they just sent the bill.
