What is the point of different health insurance premiums within the same Canton?

Hi,
I do somehow get the justification for difference between Cantons ( There are many reasons indeed). But within the same Canton, one can indeed choose their family doctor (and hospital when necessary) from anywhere in the Canton, no matter in which Gemeinde/Commune they live (right?). So why are there price differences, sometime non-negligible, between parts of the same Canton?

Without knowing the official reason for it, my guess would be their clever mathematicians have some clever formulas for that

The rates are based on post code, or regions within a Kanton. I think that if you live in a more urban, crowded area the statistics suggest higher usage so the closer you are to a city or a glut of doctors, the higher your rate. Unless iím wrong and itís higher if youíre in a less urban area.

Of course they are. My question is why.
Shouldn't it actually become "cheaper" when more doctors are available?

C’mon. Doctors and clinics and hostpitals and pharmacies do not compete on price. Almost all will bill the maximum amount the insurance companies will pay, some will charge more.

This has nothing to do with what insurance pays out. Itís the premium patients pay.

Iím pretty sure that in cities you have access to more specialty care, more expensive and more heavily utilized. Healthcare utilization just doesnít work like a free market good.

I would guess that people in rural, isolated areas simply do not "consume" medical services at the same rate as someone living in a city. Therefore, they are "entitled" to lower rates (?).


Personal example: We live in a village in Berner Oberland. The nearest paediatrician is 35 km away (in Interlaken). Obviously, for minor ailments, we have to make do with Medgate (telemedicine provider). We end up costing much less than a comparable family with two small children in an urban area...
