Health insurance deductible based on date of treatment or date of billing?


I had surgery a few months ago and I'm still waiting on the bill from the hospital. Since I've already had significant costs related to my surgery (up to just under my excess), I'd really like the final bill to be considered as part of this year's health expenses, otherwise I'll have the full excess to pay.

So, does anyone know whether health insurance deductibles for a given year are calculated based on the date of treatment, or the date of issuance of the bill? It could make a big difference to my healthcare costs.
