As is traditional for surveys I started at the bottom and worked up, so I started by not having any idea what you were talking about. Which is what successful Market Researchers aim for. So, an excellent survey!

You forgot the introductory line to ensure that all answers are relevant to the (undefined) topic in-hand. Your spelling is also satisfactory, which is clearly unacceptable.
surveymonkey is great. I have used it very successfully for several surveys. Designing them requires a lot of thought, iteration and feedback before going live.
Yours is patently (sic) an improvement of the one you parody.

The order of filling questions wasn't given...so i filled 2 questions from bottom then rest from the top. Hope one of the approaches work for you ;-)
