Normally it is a 2 step process.
your employer has to apply for the prolongation of your work permit.once this is approved, you will have to submit the B residence permit at your local gemainde. check with your employer if he has done part1 and got the approval.
for the 2nd part, if u have not received the prolongation form until now, you can go to your gemainde and ask for one. the gemainde will give the same forms but without your details printed and the gemainde can fill them for you bylooking at your current permit.
remember yiur b permit is both a work and residence permit. the work part has to be applied by your employer and the residence part will be applied by you.
the authorities match both of them when extending the new b permit.

The 1st part is a bit unclear for me, last time the process was as follows: I got a form by post from Migrationsamt, filled it in, got it stamped by my employer, brought it to Gemeinde, after approx. 6 week got a letter from Gemeinde that my permit is ready, came to Gemeinde and picked the permit up.
I'm not sure if my employer applied for prolongation, I think in this case I wouldn't need to get a confirmation (stamp) from employer once again, although who knows, this part is hidden from us.
And yes, I'm not EU and have indefinite work contract.
The 2nd part is exactly what I need. The same information I got from my colleagues, some of them also confirmed, that there are delays from Migrationsamt side.
So, today, I went to Gemeinde and got empty forms, which I filled in by hand, basically I had to write my name and address, the rest I need to fill anyway. Then I got a stamp from my employer and brought papers back to Gemeinde.
Also I found a deadline, it was written on the form: the form must be submitted no later than 2 weeks before expiry of the permit.
However it is still unclear, why they tried to force me to wait until forms come by post, if there is possibility to get it from Gemeinde. I made few copies of blank forms for the next year ))
