Swiss Passport [Citizenship] Form Question

Hello, I am just looking into filling in the Swiss Passport form. There is one point I don't understand.

I have to supply various things. One of them is:

Erklärung über die Erfüllung der Einbürgerungsvoraussetzungen ab dem 12. Geburtstag

Declaration of the fulfillment of the naturalization requirements from the 12th birthday

I am applying as a single person not a family, but I am guessing I have to supply something, because it doesn't say "falls relevant"

Any idea what this actually is? Do I just have to sign an form? If so what form?
